Aircraft Accident Investigation Board (AAIB) finding: fatal fall during heli hoist operation was caused by improper harness attachment

A civilian volunteer who fell to his death while he was being lowered from a HH-60G operated by the California Air National Guard’s 129^th Rescue Wing (RQW) was not properly secured to his harness, according to a report released by the US Air Force Air Combat Command Aircraft Accident Investigation Board (AAIB).
